Contact Lens Refill Requests Fill out the following form to request a contact lens refill from Dr. Clouse.  All prescription refill requests must be accompanied by a valid contact lens prescription.  If Dr. Clouse does not already have a valid contact lens prescription (good for 1 year from the time of your exam), fax your prescription to 503-944-5471.  You will be contacted by the office to confirm your order before it is processed.
